Testing Certification:  

I certify that my child tested negative for COVID-19 using an FDA approved/ authorized test that was taken upon symptom onset, or on or after day 5 from a positive COVID-19 test.

My student has remained fever free without the use of fever reducing medication and has experienced improvement in the following COVID-19 like symptoms in the last 24 hours:

• Fever or chills (No fever greater than 100 without the use of fever reducing medication)
• Diarrhea
• Loss of taste or smell
• Shortness of breath (if shortness of breath is a chronic condition), or other respiratory symptoms
• Muscle aches or severe fatigue
• Nausea or other GI symptoms except for diarrhea or vomiting
• Headache
• Chills
• Shortness of breath (if not a chronic condition)
• Vomiting
• Cough
• Sore throat
• Congestion
• Runny nose

Mi estudiante ha permanecido sin fiebre sin utilizar medicamentos antifebriles y ha experimentado una mejora en los siguientes síntomas similares a los de la COVID-19 en las últimas 24 horas:

• Fiebre o escalofríos (sin fiebre superior a 100 sin el uso de  medicina para bajar la fiebre)
• Diarrea
• Pérdida del gusto o del olfato
• Dificultad para respirar (si la dificultad para respirar es una condición crónica) o el estudiante tiene otros problemas respiratorios
• Dolores musculares o fatiga severa
• Náuseas u otros síntomas gastrointestinales excepto diarrea o vómitos
• Dolor de cabeza
• Escalofríos
• Dificultad para respirar (si no es una condición crónica)
• Vómitos
• Tos
• Dolor de garganta
• Congestión
